What to Expect at Your Appointment

To help make check-in easier, we provide online patient forms you can complete before your visit. At your first appointment, Dr. Jason Rogers or Dr. Leonard Tyko will review your health history and current medications with you. If your referring doctor has not already provided imaging, we will take digital X-rays in our office. Dr. Rogers or Dr. Tyko will then explain your recommended treatment plan and answer your questions. Depending on your needs, treatment may be completed the same day, or you may be scheduled for a follow-up visit.

What to Bring to Your First Visit

For your initial appointment, please bring the following items:

A surgical referral slip from your dentist and any current X-rays.
A list of medications you are currently taking or being administered.
Information about any medical conditions you have.
Your dental insurance cards, so we can assist with billing.
A parent or guardian if you are under 18.

Having these materials ready helps us deliver the best care possible and keeps your visit moving smoothly.

Scheduling Your First Visit

Appointments are available Monday through Friday, from 8 a.m. to 5 p.m. Surgical procedures are usually scheduled in the morning. If you have facial pain or a dental emergency, we will do everything we can to see you as soon as possible. Please plan to arrive a few minutes early for your visit. If you need to change your appointment, we ask that you notify us at least 48 business hours in advance.
